For Island Patients
The Seacoast Care Collaborative is a part of Maine Seacoast Mission’s Island Health program in partnership with MCD Global Health. The program provides everything you need to attend a healthcare appointment in your own home.
If you would like to request your island’s backpack, please use the calendar below. You will receive a confirmation if you have successfully made a reservation.
If you have chest pain, trouble breathing, or a serious injury, call 911 right away.
If you need emotional support, reach out to the national mental health hotline by calling 988.

Urgent Care via Island-based Telehealth
You can connect with a healthcare provider or counselor right from home. Use virtual urgent care for minor, non-emergency health needs like colds, rashes, infections, or sprains.
Call or Message Your Primary Care Provider (PCP)
- Most PCP offices have after-hours phone lines.
- You can call the office or send a message through your patient portal (like MyChart).
- They may offer a same-day or virtual visit, or help you decide your next step.
Check Your Health Insurance
- Call the Member Services number on the back of your insurance card.
- Ask: “Do I have access to virtual urgent care?”
- Many plans offer 24/7 video visits for minor issues like colds, rashes, or medication questions.
Try a Virtual Urgent Care Service
A co-pay may be required, depending on your health insurance plan.
Preparing for Island-based Telehealth Visit
Please read these tips to help you prepare for your appointment.
Get ready early

- Find a quiet, comfortable, private space for your telehealth appointment if possible.
- Make sure you have plenty of light in the room so your provider can see you well.
- Set up all your equipment early – you may not need to use everything in your telehealth backpack, get if you know you need to show the provider something, organize in advance.
- If you need to show something on the skin or in the mouth, plug in the Telemedicine Wand.
- If you need to show something in the ear, plug in the otoscope.
- If you have questions about breathing, know where the stethoscope is located (the provider may only need the microphone on your computer).
- If you are hard of hearing, plug in the headphones. If there is more than one person joining the visit, use the round speakerphone.
- Make sure your device is connected to the internet.
- Have a list of all of your prescription medications, and your log of recent blood pressure readings and/or blood glucose levels if applicable.
- Write down any questions you want to ask your healthcare provider.
- If you have children in the room, we suggest that you prepare an activity or toy to help the children stay busy so that you can focus on the provider as much as possible.
Sign on early

- Log into your account early so that you know how to find the meeting link and complete any consent forms before the visit starts.
- Many telehealth appointments are hosted through the patient portal for the organization you are seeing – make sure you know how to log on.
